The tier is a bill-splitting rule, not a quality rating

Actuarial value is the share of covered medical costs a plan pays across everyone enrolled in it. That is the whole meaning of the metal name. Two plans from the same carrier on different tiers can share the exact same doctor network.

Bronze

Plan pays about 60%

Lowest monthly premium, highest deductible. Built for people who mostly need protection against a catastrophe.

Silver

Plan pays about 70%

The benchmark tier, and the only one that unlocks cost-sharing reductions if you earn under 250% of the poverty level.

Gold

Higher premium, much lower deductible. Often the right answer if you take regular medication or expect a procedure.

Platinum

Highest premium, lowest out-of-pocket costs. Worth modelling if you have a chronic condition or a planned surgery.

Catastrophic plans

Available if you are under 30, or at any age with a hardship or affordability exemption. The deductible equals the annual out-of-pocket maximum, and three primary care visits plus preventive care are covered before it applies.

Premium tax credits cannot be applied to a catastrophic plan, so a subsidised Bronze plan is usually cheaper.

Networks, in one line each

HMO
You pick a primary care doctor and get referrals for specialists. Out-of-network care is not covered except in an emergency. Usually the cheapest premium.
PPO
You can see specialists without a referral and go out of network at a higher cost. The widest access, and the highest premium for it.
EPO
No referrals needed, but no out-of-network coverage at all outside an emergency. A middle path between HMO and PPO.
